Please note that individual total compensation for this position will be determined at the Company's sole discretion and the wage range for this role considers a wide range of factors including but not limited to skill sets; experience and training; licensure and certifications; and other business and organizational needs. At SNAHC, it is not typical for an individual to be hired at or near the top of the range for their role and compensation decisions are dependent on the facts and circumstances of each case. A reasonable estimate of the current range is $82,450.68-$111,550.92
Position Summary:
The Care Team Nurse (NCT) reports to the Nurse Administrator and provides general nursing care and primary care team leadership, working collaboratively with physicians and multidisciplinary team members. The Care Team Nurse will promote and restore patient’s health as they provide, delegate, evaluate and coordinate nursing care for all patients. This individual may oversee and participate in clinical care and case management of patients.
Essential Functions:
- Assesses, plans, directs and evaluates total nursing care as determined by the patient’s age related physical, psychosocial, and cultural needs in accordance with established standards, policies and procedures of the health center.
- Promotes patient's independence by establishing patient care goals.
- Assists provider with examination and procedures as requested.
- Provides case management for complex, high needs patients by serving as a patient advocate, patient resource and contact person for patient treatment coordination.
- May provide health related counseling on the prevention of communicable diseases; coordinate health programs with outside organizations and agencies; introduce general health related information and instruction to individuals, families and community groups.
- Assesses and evaluates patient’s response to treatment and medication.
- Keeps accurate patient records; maintain confidentiality at all times.
- Properly prepares and administers immunizations, vaccines, and therapeutic injections.
- Assists in research and investigation of communicable diseases when requested.
- Coordinates referrals to IHSS.
- Serves as a resource person to other staff in the area of one’s own clinical expertise.
- Reports communicable disease to the county.
- Complies with all state and federal laws and regulations, as they pertain to position including; HIPAA, sexual harassment, scope of practice, OSHA, etc.
- Other duties as assigned.
Skills and Abilities:
- Excellent computer skills, preferably with Windows, including Microsoft Office Suite
- Excellent telephone and communication skills
- Must possess excellent organizational, writing, and verbal skills.
- Ability to work independently, set priorities, and work well under pressure.

Minimum Qualifications:
- Valid California RN License.
- Experience with an electronic health record system.
- Ability to chart visits comprehensively.
- Computer literacy including word processing, office software, patient processing, and electronic mail systems.
Preferred Qualifications:
- 2 years of experience working in community health as a member of a multi-disciplinary team preferred.
- Experience in an FQHC setting
- Knowledge of traditional, cultural and spiritual practices of the diverse AI/AN community, as well as ability to work with other racially, culturally, ethnically, and financially diverse populations.
- NextGen experience preferred.
