Pay Range: Hourly – Min: $28.00 – Max: $34.00 (based on qualifications and experience)

Job Summary:

The Nurse Care Coordinator works in collaboration and continuous partnership with chronically ill or “high-risk” patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach

Must foster a culture of patient and family centered care, service, quality and safety excellence, while improving patient experience, employee experience and community relations

Primary Duties & Responsibilities:

  • Promote timely access to appropriate care
  • Increase utilization of preventative care
  • Reduce emergency room utilization and hospital readmissions
  • Increase comprehension through culturally and linguistically appropriate education
  • Create and promote adherence to a care plan, developed in coordination with the patient, primary care provider, and family/caregiver(s)
  • Increase continuity of care by managing relationships with tertiary care providers, transitions-in-care, and referrals
  • Increase patients’ ability for self-management and shared decision-making
  • Provide medication reconciliation
  • Connect patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing
  • Serve as the contact point, advocate, and informational resource for patients, care team, family/caregiver(s), payers, and community resources
  • Work with patients to plan and monitor care:
    • Assess patient’s unmet health and social needs
    • Develop a care plan with the patient, family/caregiver(s) and providers (emergency plan, health management plan, medical summary, and ongoing action plan, as appropriate)
    • Monitor adherence to care plans, evaluate effectiveness, monitor patient progress in a timely manner, and facilitate changes as needed
    • Create ongoing processes for patient and family/caregiver(s) to determine and request the level of care coordination support they desire at any given point in time
  • Facilitate patient access to appropriate medical and specialty providers
  • Educate patient and family/caregiver(s) about relevant community resources
  • Facilitate and attend meetings between patient, family/caregiver(s), care team, payers, and community resources, as needed
  • Cultivate and support primary care and specialty provider co-management with timely communication, inquiry, follow-up, and integration of information into the care plan regarding transitions-in-care and referrals
  • Assist with the identification of “high-risk” patients (the chronically ill and those with special health care needs)
  • Attend all Care Coordinator training courses/webinars and meetings
  • Provide feedback for the improvement of the Care Coordination Program
  • Coordinates all diagnostic orders in conjunction with workflow. Accurately documents tracking of orders daily and throughout the order cycle.
  • Uses reports daily to manage the status; schedule appointments, obtain prior authorizations, notify patients, track and monitor as defined in workflow; follow up on pending or incomplete prior authorizations and orders001
  • Licensed/Certified Staff:
  • Performs office nurse duties within scope of licensure as needed
  • Licensed staff will need to be privileged and credentialed as an Other Licensed or Certified Health Care Practitioner (OLCP).
  • A competency check must be completed at the time of the credentialing process

Job Qualifications

Knowledge, skills, and ability:

  • Bilingual Preferred
  • Must be familiar with community agencies, ability to work independently with internal and external customers
  • Strong written and verbal communication skills
  • Able to take and follow through with delegated tasks and accountability
  • Ability to demonstrate excellent customer service and strong cultural competency
  • Ability to adhere to strict confidentiality guidelines
  • Medical terminology

Experience:

  • 2+ years experience working with patients with chronic conditions, Prenatal/Perinatal patients, Children & Families. Prior experience with care coordination/care management in a medical setting. Prior experience with managing competing priorities. Professional work experience will be considered

Required Education:

  • Bachelor of Master's degree in Nursing

Working Conditions & Physical Environment: While performing the duties of this position the employee travels frequently by vehicle for home visits. Position involves frequent walking, standing, sitting, talking/hearing, bending, requires use of nursing equipment, some use of general office machines including computer, copier, printer, fax, phones, calculators, and shredder.
