JOB SUMMARY: The Navigator Case Worker provides comprehensive, holistic support services to vulnerable immigrants, assisting them in identifying and accessing needed services. The position works with clients to assess needs, develop a plan, and operationalize the plan, leading to positive social outcomes.
About the Newcomer Network: The Newcomer Network is a new initiative of Catholic Charities Washington, DC, developed in response to the needs of the large and diverse immigrant population within the Agency’s program area. The network is tasked to assist immigrants with legal services and connect them to comprehensive supports through a common intake and holistic case management. The Network also leverages the Agency’s parish network to achieve a broader impact and meet immigrants where they are.
ESSENTIAL DUTIES and RESPONSIBILITIES:
- Perform individual casework through conducting needs assessments with clients and developing goal plans in accordance with the needs and wishes of the clients.
- Make referrals to service providers and collaborate with service providers and advocate for clients as needed by maintaining prompt and regular contact with clients in the ongoing provision of services.
- Conduct monthly follow-up with clients for up to one year to evaluate and document service success and outcomes.
- Collaborate with the Newcomer Network team in the provision of services by actively participating in team meetings, reporting on the progress of cases, and working with colleagues to problem solve and develop solutions.
- Document outputs and outcomes by promptly administering initial assessment instrument and periodic follow up assessments, tracking referrals and their outcomes, entering data into designated computer systems, and completing and submitting reports.
EDUCATION and EXPERIENCE:
- AA degree.
- Three (3) years’ experience providing services to vulnerable populations.
SKILLS and COMPETENCIES:
- Strong spoken communication skills in English and in Spanish.
- Driving is required. Must have a valid driver’s license and clean driving record.
