Under the general supervision of the CPO, responsible for directing, documenting, and coordinating the Moms & Babies First Program which provides African American pregnant women with support and education, assistance accessing early and consistent prenatal care, and community referrals.

Moms and Babies First Program Manager will support Community Health Workers in providing home-based educational and support services to enrolled expectant mothers. The person in this position will provide supervision to Community Health Workers in planning and implementing home-based program activities for families, compliance with Ohio Department of Health performance standards and implementation of the home visiting curriculum. The Manager will ensure Community Health Workers maintain organized files and thorough documentation of services provided.

Major Responsibilities/Activities:

Provide Community Health Workers with training, modeling and coaching to ensure the delivery of responsive services that are in line with the Ohio Department of Health performance standards and Partners For Healthy Babies curriculum.

Ensure Community Health Workers maintain accurate, up to date records of home visits, child progress, parent contact, parent goals, lesson plans, data entry into the State database. Ensure all child/family files are up to date and accurate by conducting and documenting a minimum of a quarterly chart audit.

Ensure Community Health Workers are connecting families to community resources where appropriate.

Balance a client focused service culture with a culture where each employee assumes personal responsibility for agency financial viability and exercises the responsibilities of the position according to the budgetary constraints imposed by management.

Minimum Requirements:

This position requires certification as a Community Health Worker. Bachelor's Degree in Related Field will be considered with successful completion of Community Health Worker Training required. Supervisory experience preferred. Position requires strong interpersonal, organizational and written skills and the ability to function effectively in a team approach to service delivery. Must possess a valid license to operate a motor vehicle in Ohio and carry automobile insurance at the agency-established or contract required minimums.
