Position: Full Time MMQ Assessment Nurse

Location: Vantage at South Hadley

Supervisor: Facility Director of Nursing and Regional Director of Clinical Reimbursement

Pre-requisites:

• Registered Nurse or Licensed Practical Nurse
• Minimum of two (2) years’ experience with MMQ’s.
• Powerful knowledge of all applicable payer sources and related participation requirements.
• Exceptional assessment and communication skills.
• Ability to teach and monitor key documentation by others.
• Monitors completion of MMQ documentation by other disciplines within timeframes prescribed by regulatory guidelines.
• Participates in care plans with resident and interdisciplinary team.
• Analyzes data and advises staff on acceptable standards of documentation at all levels.
• Participates in Division of Medical Assistance audits as needed and monitors accuracy of nursing documentation.
• Great team player.

SSENTIAL DUTIES AND RESPONSIBILITIES:

• Responsible for the accurate completion of Management Minutes Questionnaire (MMQ) for all residents as needed: Semi-annually, upon Conversion to Medicaid or Medicaid Pending, Admission to Medicaid and with Significant Change.

• The MMQ Coordinator will work with team members to assure good communication of resident needs and goals. Observe for and communicate any changes in resident care to appropriate team members.

• Completes full chart reviews/audits of all documentation to ensure accuracy of care delivered as part of the MMQ completion process to ensure the facility is accurately reimbursed for care provided.

• Monitors certified nursing assistants’ documentation for completeness and accuracy.

• Monitors nursing treatment sheet documentation for completeness and accuracy.
