WHAT DOES THE JOB LOOK LIKE?
This position provides Targeted Case Management services for a diverse group of children. These services are available for MN Medicaid recipients, including adults with Serious & Persistent Mental Illness (SPMI) and children with Serious Emotional Disturbance (SED). Services are designed to assist our clients in gaining access to needed medical, social, educational, financial, and other services necessary to ensure stabilization of functioning and to meet mental health needs.
There are four primary responsibilities of a Case Manager. These include: 1) Assessment, 2) Treatment Planning; 3) Referral & Linkage; and 4) Monitoring and Coordination. The Case Manager develops a functional assessment and Individual Community Support Plan, designed to meet the client's needs. The Case Manager makes needed referrals, assists clients in obtaining needed mental health and other services, coordinates the individual's services, and monitors the effectiveness of services that are being provided for the client. Services the Case Manager may assist in accessing and monitoring include individual, group, or family psychotherapy; medication management; day treatment; home-based therapeutic services; education or health services; and housing and related needs.

Solutions is looking for a team player who is passionate about our mission! A qualified individual would have excellent “people” skills. This individual must meet state criteria for classification as a Case Manager, i.e., hold a bachelor's degree in a behavioral science or related field along with one year of supervised experience in the delivery of services to persons with mental illness, or alternatively receive required training from the Department of Human Services upon hire. An ideal candidate would have solid knowledge of the available services in the community. They would be versed in identifying stages of change and have foundational skills in motivational interviewing. They would strive for cultural competence. We are looking for someone who can work independently and manage their schedule. This individual must pass a criminal background check, possess a valid driver’s license and maintain current automobile insurance.
