The Provider & Member Services Outreach Specialist will work as an integral part of an interdisciplinary team, using Provider Data Management Reports reports and practice registries to proactively identify patients due or over-due for specific health services, contacts the patients, motivates the patient to complete the recommendation, and assists in scheduling appointments. The Outreach Specialist is a valuable resource for analyzing provider panels, care gap reports, and providing outreach to patients overdue for services. It is essential that the Outreach Specialist maintain a patient focus, shares a passion for patient-centered care, and assist in meeting organizational and efficiency goals. The Outreach Specialist will also serve as a liaison to physician practices for BHQN‘s CIN and ACO attributed patients responsible to schedule appointments, maintain and track patient status throughout the care continuum in order to close the gap for care coordination.

Estimated pay range for this position is $54165 - $70414.5 / year depending on experience.

Qualifications:


Degrees: Bachelors Additional Qualifications: Ability to work independently with strong organizational, communication and interpersonal skills, with significant attention to detail for completion of both verbal and written external communications. Bilingual in English and Spanish fluency required. Data entry skills required with knowledge of Word and Excel. An additional 4 years of experience can substitute for required education.
