MEDS TO BEDS/PATIENT CARE COORDINATOR - OUTPATIENT HOSPITAL PHARMACY
Maxor National Pharmacy Services Corporation is seeking a highly competent, customer service oriented, full time person to work as a Patient Care Coordinator in the Meds-To-Beds program, in this fast-paced, outpatient pharmacy at St. John's Episcopal Hospital Outpatient Pharmacy.

JOB SUMMARY:
- Interact with patients to obtain permission to fill discharge medications, ensure patients are eligible for services, have method of payment, and are properly educated.
- Responsible for the procurement of all patient’s information and prescriptions prior to discharge.
- Coordinate with clinical care team, case management, and other staff to ensure medications are delivered without delaying discharge.
- Enters medication orders from doctors' order sheets/prescriptions and enters correct billing data as required. Completes and files appropriate paperwork to ensure payment for and delivery of pharmacy supplies. Monitors lost charges.
- Schedules and coordinates pharmacist consultation review of patient’s prescribed medications to drive medication adherence and education.
- Deliver medications to patients and/or nursing units under the direct supervision of a licensed pharmacist as part of our Meds to Beds service.
- Educates potential patients about services provided at time of discharge and articulates how patients may utilize pharmacy services to obtain full benefits of the high-quality prescriptive care.
- Identifies and resolves insurance billing adjudication; including facilitating prior authorizations (PAs), and assisting in locating financial assistance for patients as needed.
- Conducts weekly follow up calls with hospital patients post following discharge to ensure aftercare plans have been met.
- Work in a hospital and pharmacy setting and builds strong relationship with patients, hospital staff, pharmacy team as well as other clients and stakeholders aligned with the business group
- Perform other job-related duties as assigned.
Experience:
A combination of prior pharmacy experience in a high volume, fast paced pharmacy, along with prior Patient Care Coordinator experience is a PLUS
Education:
High School Diploma or equivalency
