AJL Nursing Services, LLC dba Champion Home Health Care is looking for a Medical Social Worker to join their team. The Medical Social Worker will provide services to patients in their homes (Brevard County) in accordance with physician orders.

Position Responsibilities:

1. Assist in the admission process of the patient by performing an initial evaluation and assessing the patient’s psychosocial status.

2. Assist in developing and implementing the interdisciplinary patient care plan.

3. Observe, record, and report changes in the patients emotional and social factors that affect their care and response to treatment.

4. Consult with the attending physician concerning alteration of the plan of treatment.

5. Maintain and submit written clinical records, that include the initial evaluation, the care plan, and daily notes.

6. Confirm the scheduling of visits with the Director of Nursing to facilitate the coordination of other staff visits.

7. Participate in activities and committees when appropriate to include staff development activities and in-service education.

8. Supervise the social work assistant.

9. Participate in discharge planning.

10. Assess the patient’s ability to cope with his or her social and health problems.

11. Act as a consultant to the members of the health team by helping them understand the social, emotional, and environmental factors related to the patient’s health.

12. Provide rehabilitative and supportive casework.

Qualifications:

1. Master’s Degree from a school of Social Work approved by the Council of Social Work Education.

2. Must have or be in the process of acquiring certification from the Academy of Certified Social Workers.

3. Two years’ experience with at least one year of experience in a health care setting.

4. Must have a criminal background check clearance, national sex registry clearance, and OIG clearance.

5. Must have current CPR certification.

6. Must be eligible to work in the U.S. We participate in E-Verify.

7. Must be able to pass a drug test.
