The Maternal and Child Health (MCH) Program Coordinator provides leadership, guidance, and support to develop and implement evidence-based strategies to improve maternal, infant, and early childhood health and wellness outcomes, including but not limited to maternal mortality and the promotion of healthy mothers and children.
In collaboration with Pediatric and OB Providers, the Maternal Child Health Program Coordinator provides care for expecting and postpartum mothers, infants and children. The MCH Program Coordinator works closely with DePaul Community Health Centers and LSU high -risk obstetric and pediatric and prenatal providers to ensure screenings, referrals and follow up care is completed timely. The ideal candidate will have experience with maternal and pediatric health. The individual filling this role will work with a diverse background and engage all levels of staffing. A key responsibility is to work with patients in providing Remote Patient Monitoring for prenatal and postpartum patients.
Essential Responsibilities
- Work closely with all prenatal providers and other members of the team at all locations to coordinate specific aspects of evidenced based care for prenatal-postpartum patients. Collaborate with the team to develop a care plan that addresses identified problems.
- Serve as the DCHC primary liaison at DePaul Community Health Centers for Maternal Child Health related research projects and Maternal Child Health initiatives and programs
- Provide general administrative assistance and clinical support for DePaul Community Health Centers Maternal Child Health programs, including supporting the MOM Power, AIM and Ascension Health Maternal Remote Patient Monitoring programs.
- Meet regularly with Clinical teams and other special program staff to support MCH activities and achievement of shared program goals and grant deliverables.
- Integrates Behavioral Health and SDOH screens and early intervention,
- Assist with implementation of postpartum bundles to improve postpartum health and interconceptual care.
- Ensures the completion of 2 y/o immunizations and developmental screens and early childhood interventions
- Provide patient education as needed and assists in the identification of culturally competent educational resources.
- Provide administrative assistance to the prenatal provider team by following up on specific patient concerns or patient issues
- Participate in organizational activities that facilitate internal coordination of efforts, interagency collaboration, and stakeholder partnerships, including working with other MCH focused organizations to implement programming.
- Develop and maintain tracking log of prenatal patients
- Work with Community Health Navigators and other members of the team to collect data and support the management of referrals and screening for Social Determinants of Health
- Assist Executive Director with preparing reports for grants and program deliverables
- Perform other related duties as required
Qualifications:
- Registered Nurse, LPN or equivalent healthcare credentials preferred
- Qualified candidate may also have 2 to 4 years of related experience in working with prenatal patients in the clinical setting
- Public Health experience a plus
- Demonstrated strong interpersonal skills and respectful interactions with other agencies and project leaders. Must interact well with staff and patients and have a friendly personality.
- Demonstrated experience performing administrative duties with accuracy and works well without immediate and constant supervision.
- Ability to multitask and prioritize needs of multiple programs
- Must be highly organized and detail oriented
- Ability to maintain and improve upon tracking system for prenatal, postpartum and pediactric patients.
- Ability to communicate effectively both orally and in writing with an attention to detail and professionalism in all communications.
- Must possess a collaborative spirit and employ ways to be helpful to the MCH team
- Bilingual English/Spanish preferred
- Proficient in Google Workspace
- Ability to learn and navigate new data systems for grant reporting and research
- Demonstrated commitment to, and interest in learning more about, maternal child health.
