Join our Award-Winning Team today! Currently seeking a Shift Supervisor at our RoseWood Village Assisted Living at Greenbrier Community.

Wellness Coordinator who will be full time, primarily on Day Shift, and primarily Monday through Friday although there may be an occasional need for weekend scheduling. This position will be responsible for managing the clinical communication with health providers and family members; managing clinical documentation, and wellness follow-up. We have RMA supervisors who will handle the day to day processing of new orders and managing the nursing assistants as well as passing meds. We feel that with a nurse in place to handle the clinical communications and keep documentation in order, it will be a great team.

Responsibilities

• Assists in the welcome and orientation of new residents.
• Ensures all admission documents are complete, accurate and received.
• Participates in completion of Universal Assessment Instrument (UAIs).
• Ensures resident charts are current and organized and required documentation is completed.
• Observes and/or follows-up on changes in residents’ conditions. Reports changes to physician and families; documents all actions taken.
• Functions as liaison with families, physicians, and related health care agencies regarding care/ health and wellness issues for residents.
• Foster teamwork and have a positive impact on team members, residents, and visitors.

Qualifications

• Must be 21 years of age or older.
• Must read, write, speak and understand English.
• A Registered Nurse/Licensed Nurse in good standing, meeting all applicable federal and state licensure, is preferred.
• Demonstrated leadership and supervisory skills.
• Knowledge or willingness to learn computer programs used in the community.
• Previous experience in a healthcare-related community or equivalent preferably assisted living.
• Must be able to pass a criminal background check.
