BrightStar Care of Westfield and Summit is currently hiring per diem LPNs to staff a local hospice facility in Union County (Scotch Plains & Elizabeth). The LPN will provide comfort and optimize the quality of life of terminally ill patients while providing skilled nursing care following doctors orders.

Responsibilities

• Provides skilled nursing care and ongoing support where patient/family caregiver is requiring care as assigned by the RN and ordered on the Plan of Care
• Initiates and sustains, as needed the implementation of orders for medication and treatments as prescribed by the physician
• Responds to all needs of terminally ill patients and their families
• Carries out pain and symptom management plan, keeping patient as comfortable as possible
• Applies appropriate preventative and therapeutic nursing procedures and techniques
• Reports any changes in condition, records pain/symptom changes
• Demonstrates knowledge of medications; responsible for ordering and reordering medications
• Provides and maintains a safe environment
• Assists in providing continuity of care
• Assist patient with ADLs, teaches patient and family selfcare techniques as appropriate
• Provides medication, diet and other instructions per doctors orders
• Supervises care of subordinate care providers
• Completes accurate clinical notes

Qualifications and Requirements

• Unencumbered license as a Practical Nurse in New Jersey
• One (1) year of nursing experience required; three (3) years preferred.
• Certified CPR and negative TB skin test or chest x-ray
• Physical Exam signed by medical professional, clearing you for work
• Up-to-date Covid vaccine including booster shot
• Licensed driver with valid automobile insurance
• Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds
• Understands philosophy, principles of death/dying
• Understands hospice and palliative care philosophies
• Possesses excellent observation, good nursing judgement and communication skills
• Self-directed with the ability to work with minimal supervision
• Posseses good physical stamina and mental health

Pay: $32.00 per hour
