Your Specific Responsibilities:

The Licensed Practical Nurse position is an employee of the Florida Department of Health in Hendry County (FDOH-Hendry), represents the agency, its mission, and values. This is a sensitive position within the Ryan White program responsible for assisting the Physician, Advanced Practice Registered Nurse (APRN), and r Registered Nurse (RN) in providing comprehensive primary medical care and essential support services for people living with HIV (PLWH). The incumbent is expected to provide high quality customer service and clinical excellence and independently perform assigned duties while ensuring thorough and accurate documentation of charts and medical records. In addition, the incumbent will be responsible for ordering items from state approved vendors via My Florida Market Place (MFMP) and/or other approved vendors and play a role in the Quality Assurance/Quality Improvement (QA/QI) program ensuring Health Resources and Services Administration (HRSA) grant deliverables are met or exceeded. Position reports directly to the Advanced Practice Registered Nurse and is headquartered in LaBelle. Travel to and from clinic sites: 1) La Belle and Clewiston is a requirement.


Works with HIV clients in the clinic setting with labs, medications, and assisting the clinician as needed. Keeps accurate records and report of duties performed. Coordinates and participates in the Department of Health’s Performance Improvement programs. Must be able to multi-task with attention to quality outcomes. Assures a safe and healthy environment, including adherence to OSHA guidelines for prevention of exposure to blood borne pathogens, utilization of universal precautions, and communicable disease surveillance and education to prevent the spread of infections. Maintains confidentiality of all records, client interactions and services according to HIPPA privacy policies. Completes follow-up when indicated by abnormal lab results, missed appointments or other concerns as deemed necessary on an individual basis. Provides direct nursing services, per protocols, including, but not limited to, patient intake, interview, patient flow, phlebotomy, vitals, history, specimen collection, assisting medical providers with procedures, and packaging and mailing of hazardous materials. Educates and counsels clients on lifestyle, compliance, and health practices. Administers medication with concurrent education. Assists with patient referrals to specialty and sub-specialty services and tracking of these referrals. Ensures exam rooms are maintained and faciliates ordering of supplies and equipment. Documents accurately and thoroughly in medical records per program guidelines. Understands and uses HMS and CAREWARE software for data input. Reports any abnormalties to the appropriate medical professional and/or program manager immediately. Maintains schedules including recalls and, as needed, referrals.

Administrative

Assists the Program Manager and providers with the administrative functions of the Ryan White Part C grant program. Compiles and enters program data into CAREWARE, HMS, or other software programs and generates data reports as requested. Ensures accuracy of RSR data. Participates in the organization and flow of patient charting. Maintains and updates protocol manual and develops clinical brochures. Provides clerical support for Continuous Quality Improvement and Quality Assurance activities and special projects. Schedules meetings, contacts participants, and takes meeting minutes.

Quality Assurance and Quality Improvement

The incumbent will participate with the design, develop, implement and manage the QA/QI program, including assisting division director with the establishment of performance measures and benchmarks, current performance and trends, objective selection methodology for testing performance, performance improvement initiatives, and reporting methodology. Design, develop, implement, and manage a program for evaluating providers in the community and the quality of volunteer care provided to citizens. Work cooperatively to ensure quality of care and service delivery. Assists the Program Manager with special assignments, research, and report preparation. Learns new skills to assist with clinic operations. Assists with minutes in general and CQI staff meetings. Tracks 340B clients, identifies candidates, and enrolls clients.

Attends scheduled meetings, training sessions, and outreach events. Promotes public awareness related to assigned projects and develops and maintains community relationships to support client referrals. Performs other related work as assigned.


Required Knowledge, Skills, and Abilities:

  • Knowledge of nursing support practices and general public health principles, practices, and techniques.

  • Knowledge of quality improvement tools and techniques to effectively monitor and improve programmatic outcomes.

  • Ability to travel throughout Hendry and Glades counties to ensure service provision.
  • Ability to collaborate effectively with community partners.
  • Skill is the use of computers including Microsoft Office applications (word, excel, e-mail, etc.) and in the use of automated client databases such as Health Management System (HMS) or a comparable client database;
  • Ability to accurately document and maintain client medical charts (manually or electronically);
  • Ability to understand, interpret & apply applicable rules, regulations, policies & procedures;
  • Knowledge of data entry and spreadsheets;
  • Ability to accurately and efficiently capture and tabulate data;
  • Ability to communicate effectively both verbally and in writing;
  • Ability to establish and maintain effective working relationships with others;
  • Ability to plan, organize, and coordinate work assignments independently;
  • Ability to understand and apply applicable rules, regulations, policies and procedures;
  • Self-motivated;
  • Ability to develop and maintain written documentation;
  • Ability to demonstrate exceptional customer service skills.

Qualifications:

Licensure as a Florida Licensed Practical Nurse in accordance with F.S. Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.22 or 8.27 is required.


Incumbent may be required to work before, during and/or beyond normal work hours in the event of a disaster or as needed. EMERGENCY DUTY: Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or perform other emergency duties including, but not limited to responses to or threats involving any disaster or threat thereof, man-made or natural.
