SUMMARY: Licensed Practical Nurse works under the direction of a Registered Nurse in providing services and care for individuals residing in Cortland County Residential Program.
RESPONSIBILITIES: Oversees the general health and medical management of assigned consumers. This includes follow-up of health related issues and coordination with physicians, clinicians, family members, and Residential Services Program staff. Serves as the Agency Designated Supervisor to ensure on-site supervision of medical adherence to OPWDD medication regulations with the Residential Program (i.e. medication audits). Provides medical support for residential program; through assisting with arranging and scheduling of appointments, follow-ups as necessary, paperwork, and filing in accordance with the Residential system. May assist with medication order in the Residential Program and keeps R.N. appraised of any discrepancies. Assists R.N. with updating and tracking appointments, lab work and the results thereof. Upon RN direction may provide oversight and assist residential staff in implementing Nursing Plans, Nutrition Plans and any health related issues of individuals.
Accompanies individuals to significant appointments and is the liaison between individuals and physicians
Requirements:EDUCATION and/or EXPERIENCE: Valid New York State Licensed Practical Nurses License and at least six months medical experience within the developmental disabilities or public health nursing field.
