Elmwood Rehabilitation Center is a 77-skilled nursing facility. We are a FIVE-star facility by CMS. Our primary function is to offer continuous nursing, medical, and other health and social services on a 24-hour basis, under physician-directed care. Our mission is to provide quality care for our residents in a safe and comfortable environment. Job Responsibilities

You will be responsible for assisting in the day-to-day operations of the Nursing Department. This includes but is not limited to the following: -

· The role of the Infection Preventionist is to monitor and report trends in infection control by analyzing data from an electronic surveillance system.

· Responsible for assisting and identifying infection control issues and conducting both process and outcome monitoring.

· Conduct daily IP rounds and submit reports to DON and Administrator

· Coordinates outbreak investigations with the director of nursing and medical director, as needed. Analyzes data and determine appropriate course of action to address an outbreak. May recommend new policies or protocols to prevent further outbreaks.

· Responsible for providing Monthly, quarterly and annual report to the QAPI meetings.

· In-services to all staff as needed.

· Participating in the development of new or revised policies and procedures.

· All other duties as assigned by the director of nursing or the Administrator

LVN License Required

Mainly responsible for the Infection Prevention Program of the facility.'
