Other employment requirements.

New staff must have proof of meeting the vaccination guidelines per CMS guidelines. New staff must be fully vaccinated or have submitted and have an approved exemption prior to their date of hire.

• Minimum – LLMSW or LLPC with development plan for attaining full licensure within two (2) years of hire

• Current license to practice in the State of Michigan without conditions, limitations or restrictions

• Two (2) years post-master experience in social work or counseling in an inpatient setting with LMSW or LPC credentialing preferred

• CAADC (Certified Alcohol and Drug Counselor) preferred

• CAADC or Development Plan (within 30 days of employment) with MCBAP required if assigned to chemical dependency program

• Approved CPR certification

Weekends/Holidays as needed

Performs a wide range of patient care activities for an assigned caseload of patients in a behavioral care setting including but not limited to:

• Completes psychiatric admission assessments, including evaluation of psychosocial functioning, events precipitating admission, identification of patient needs, limitations, and strengths.

• Complies with The Joint Commission (TJC) and Center for Medicaid and Medicare Services (CMS) standards with regards to the quality of documentation and time frames for completions of assessments and treatment documentation.

• Coordinates the development and implementation of individualized treatment plans in consultation with the patient and other treatment team members; participates in plan reviews to determine effectiveness and revises the treatment plan accordingly.

• Gathers information regarding the patient, family dynamics, background, current situation, etc. from relevant sources. Maintains timely communication with patient’s family/significant relations and is a liaison between family, facility and community resources.

• Assist Discharge Planner with discharge plans and the rest of the team and community resources, serving as a patient advocate/case manager for securing community support/further care, provides education for patient and families/significant relations, ensures discharge packets are complete (including releases or waivers) and delivered to the appropriate aftercare providers.

• Provides intervention services including crisis intervention, individual, group, and family therapies. Identifies/makes recommendations for alternate treatment methods as necessary.

• Maintains up-to-date and appropriate information for patients records/charts.

• Utilizes the outcome measurement system and reviews for clinical improvement measures.
