PATH is seeking candidates with case management or housing navigation experience to join our Family Services Programs as a Housing Navigator.

ABOUT OUR FAMILY SERVICES
PATH's Family Services Department serves families and children who are literally homeless or at imminent risk of homelessness. The Family Solutions Center (FSC) Program provides a centralized point of access for families in need of crisis services, shelter placement, rapid rehousing, permanent housing placement, linkages with schools and employment, physical and mental health care, and other supportive services.

WHAT IS A HOUSING NAVIGATOR?
As part of the Family Services team, the Housing Navigator provides targeted housing-related case management support to help the families served with obtaining and maintaining stable housing. Responsibilities of the Housing Navigator include:

  • Reviewing intakes and assessments for families accessing services to evaluate housing needs, history, and service and support needs
  • Collaborating with clients to develop and implement individualized housing plans for each family
  • Providing shelter-based, strength-based case management services and services coordination services in the areas of and including:
    • Independent living skills
    • Housing stabilization
    • Community integration
    • Employment linkage
    • Benefits Establishment
    • Linkage to community providers for substance abuse, primary, and mental healthcare
    • Facilitating access to services required to foster housing readiness such as credit repair and legal aid.
    • Any and all other services needed to assist clients in meeting their treatment plan goals
  • Providing information and instruction to clients regarding housing related matters, such as how to complete a housing application and conduct a housing search and tenant rights and responsibilities
  • Providing ongoing case management support to assess progress and ensure treatment plan goals are met or changed as needed
  • Evaluating, determining, making recommendations for, and tracking financial assistance and preparing and submitting requests for approval
  • Collaboratively working with case management staff, housing locators, and community partner to identify appropriate housing options for clients

WHAT YOU BRING
We're looking for candidates that:

  • Maintain and execute confidential information according to HIPAA standards
  • Demonstrate strong interpersonal skills and the ability to exercise mature, professional judgement
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Work just as well independently as they do on a team
  • Exercise mature judgement, and are highly motivated, self-starting and proactive
  • Are excellent at communicating, whether in writing or verbally
  • Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment

PREFERRED QUALIFICATIONS
An excellent candidate for this role will:

  • Two (2) years of case management or housing navigation experience
  • Experience in case management, mental health, substance abuse and co-occurring disorders, and/or permanent supportive housing
  • Speak, read, and write in Spanish with business level proficiency in addition to English
  • Have knowledge of HMIS

MINIMUM QUALIFICATIONS

    • Critical Time Intervention
    • Harm Reduction strategies
    • Motivational Interviewing
    • Housing First practices
    • Non-violent crisis intervention techniques
  • Be proficient with computers and technologies used for work outputs including:
    • Microsoft Office Software
    • Strong working knowledge of Microsoft Excel
    • Database systems such as HMIS

MANDATORY REQUIREMENTS
Regardless of qualifications, for this position all prospective employees must:

  • Be able and willing to work flexible hours which may include evenings or weekends
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of full COVID-19 vaccination
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test

DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:

  • Have reliable transportation
  • A valid driver's license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATH's insurance coverage

COVID-19 VACCINATION
Effective 11/01/2021 ll require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.
