Hospice RN Case Manager for Home Care, Dekalb and Gwinnett Counties
Job description
1.Graduate from accredited school of nursing with current license in the state of Georgia.
2. Minimum of two (2) years experience in medical-surgical, oncology or community health nursing or equivalent.
3.Demonstrated ability to assist with the respond to the needs of terminally in patients in the home setting.
4. Excellent assessment skills, communication skills, and the ability to establish therapeutic interpersonal relationships with patients and caregivers.
Responsibilities & purpose of the job
As a member of the interdisciplinary team, the home hospice nurse works under the general direction of the home hospice coordinator and the attending physician. The home hospice nurse will provide skilled nursing assessments, planning, and implementation of the plan of care in order to maximize the comfort of the terminally ill patients and their families.
