Responsibilities of a Hospice RN include:
- Assess and evaluates patient needs/problems, identifies goals with patients.
- Develops patient care plans that specifically addresses identified patient problems and goals. Updates care plans on an ongoing basis
- Provides intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities as outlined in the patient IDG Plan of Care.
- Provides Skilled Nursing visits according to visit schedule
- Reports significant findings to patient’s physician and Home Office as they occur.
- Submits completed skilled nursing notes; communication notes.
- Completes communication note documenting plans for recertification were discussed and agreed upon between the physician, patient, and RN Manager.
- Effectively communicates with all members of the healthcare team.
- Acts as the patient’s advocate and as such is a liaison to assist in communicating the patient’s needs to the multidisciplinary team.
Requirements for Hospice RN includes:
- Current Indiana State license as a Registered Nurse.
- One (1) year of experience in bedside patient care.
- Hospice and/or dementia experience preferred.
- Effective communication skills
- Current Indiana Driver's License.
- Knowledge of electronic charting and basic computer skills.
