Home Health Coordinator (Bilingual English/Spanish Speaking Preferred)
We are looking for a motivated and dependable individual to coordinate home health care for our workers' compensation injured parties. This person will work with our home health providers and patients (via phone/email/text message) to coordinate continued patient care after the referral has been staffed. This position has a focus on providing prompt and timely responses and upholding the highest standards of customer service. This position requires a highly motivated individual who is comfortable working in a fast-paced, entrepreneurial environment, with rapid changes in direction.
This role reports to the Home Health Manager.
This role is remote. We hire in Utah, North Carolina, Florida, and Georgia
Daily Functions
- Following the patient's care once home health services have been staffed and ensuring that the patient is satisfied with their care
- Gathering chart notes and medical records to support the patient's care
- Matching notes with authorizations to prepare for billing
- Submit for continued authorizations
- Assisting the home health manager staff
- Keeping clients updated via email

- Be proficient with Microsoft Office Suite or related software to manage schedules, gather notes
- Be a self-starter and have the ability to work independently to complete task
- Bilingual language skills, preferrably English and Spanish
Requirements
- 1+ years of customer/patient care or support experience
- Excellent written and verbal communication skills
- Prior experience providing customer/patient service in a healthcare business is a plus
- Reliable and prompt for all shifts
- Strong people skills — you are friendly, empathetic, a good listener, and you're invigorated by constant personal interaction
- Strong passion for customer happiness, retention, and service quality
- Organized and diligent in following up on action items
