Position Summary:

The Home Health CNA will be responsible for providing personal care and related services within the patient's home under the supervision of a registered nurse or physical therapist.

Position Duties:

• Establish a relationship with the patient and patient representative(s) that promotes trust and provides emotional support
• Administer bedside and personal care such as bathing and personal hygiene assistance under the supervision of a registered nurse or physical therapist
• Assist with physical activity as assigned such as transferring, ambulating, simple exercises; encourage independence

Other Requirements:

• Must possess strong service orientation, including active listening, critical thinking skills
• Must be able to speak and write in English
• Current automobile insurance according to Company policy

Licensing/Certification/Education Requirements:

• Minimum one (1) year certified nursing assistant experience preferred
• Home health experience preferred
• Current state certified nursing assistant license

Physical Requirements:

• Job will require frequent lifting of objects up to 50 pounds
• Work requires walking, standing, bending, reaching, kneeling, pushing and pulling
