CATHOLIC GUARDIAN SERVICES
JOB TITLE: Healthcare Coordinator, Unaccompanied Alien Children (UC) Services Program
DEPARTMENT: Family Support Services
REPORTS TO: Program Director of Family Based Services
OBJECTIVE OF POSITION
To ensure that appropriate medical and dental care as required by NYC and NYC regulations and as dictated by good health care practice is provided to all children assigned to the UC program.
ESSENTIAL RESPONSABILITIES
1. Coordinates, monitors and documents the health care of the children in his/her assigned caseload. In addition, will have the oversight of the nurse that oversees TFC/Shelter. Activities include, but are not limited to:

  • Provides basic healthcare treatment in the absence of the assigned nurse.

  • Tracks periodic health and dental exams and assures all required documents have been uploaded to portal.

*

  • Coordinates referrals made to specialty clinics and follows up any recommended tests, treatments or therapies.

*

  • Follows up incident/accident reports, emergency room visits, and sick visits to ensure that the prescribed medical regimen is being carried out.

*

  • Reviews that documents for all medical and dental reports have been obtained and follows up with nurse for any specialty care the child may need.

*

  • On a case by case basis, completes or coordinates completion of forms required for specific purposes.

2. Maintains a close working relationship with the community-based health care facilities that provide the majority of health-related services to UC children. On a regular basis consults with staff of those facilities to educate them about foster care issues, to discuss and solve any problems and obstacles to UC children accessing appropriate care, and to enhance communication between all parties involved.
3. Supervises the nurses that oversees the TFC/shelter program and assures that tasks are completed and the portal is maintained updated at all times.
4. Attends training sessions (both in and out of agency) as requested by health care coordinator.
5. Other duties as assigned.
QUALIFICATIONS /SKILLS

  • LPN
  • Minimum three years of nursing experience, preferably in pediatrics.
  • Current holder of nurse?s professional liability / malpractice insurance.
  • Strong computer skills, including Word, Excel and PowerPoint
  • Proficiency in Spanish required
