Health Promotion Assistant
Location: Catholic Community Services of Utah, 224 N 2200 W, Salt Lake City, Utah, 84116
Status: Full-time, non-exempt, with full benefits
Hiring Range: $17.00-$19.00/hr DOE

Job Description:
This position will focus on the prevention and vaccination of COVID-19 for Refugee and Asylee clients. This includes the promotion of COVID-19 testing, resource assistance for those who test positive, and COVID-19 vaccine outreach and support. The position also supports the Health Promotion Coordinator in delivering state-mandated health education to clients.
Responsibilities include:
- Reach out to clients to offer support and education on the COVID-19 vaccine
- Helping clients who test positive for COVID-19 by providing health mentorship and connecting them to community resources
- Administer pre- and post-assessments to measure outcomes and follow health promotion grant requirements
- Provide transportation to COVID-19 vaccine and testing appointments
- Organize mobile vaccine and testing clinics
- Assist in creating and maintaining state and county partnerships
- Assure proper documentation of all services provided
- Keep track of clients' COVID vaccination status via USIIS data
- Assist the Health Promotion Coordinator and the rest of the health services team in other services such as health education classes, transportation to medical appointments, and providing clinic tours
Work Environment - inside/outside, office, warehouse, noise, temperature, etc.
- This position is in a professional office environment using standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Physical Demands - including bending, sitting, lifting and driving
- This is largely a sedentary role; however, some filing is required. This would require the ability to lift files, open filing cabinets and bend or stand, as necessary.
- This role includes driving locally to appointments and meetings.
Skills, knowledge and abilities:

- Be sensitive to the needs of clients, client's families, visitors, co-workers, volunteers, and other persons with whom the employee may interact.
- Recognize time as a valuable resource and respond promptly to needs of clients and co-workers.
- Consistently perform work assignments efficiently. Adhere to deadlines.
- Understand the core service requirements for newly arrived refugees and their special medical needs.
- Knowledge of Health Care Service Provider Network capable of handling special needs of refugees.
- Must maintain confidentiality regarding client's information.
- Outstanding oral and written communication skills.
- Proficiency in computer skills to complete documentation, scheduling and e-mail in Outlook.
Education, training, or experience:
- Certified Community Health Worker (CHW) preferred
- Bachelor's degree in public health or related field preferred or equivalent experience
Background Screen/BCI requirement
- Must be able to pass a national BCI/FBI background check.
Licenses, certifications and credentials
- Current Utah driver's license, proof of auto insurance with minimum policy liability limits of $50,000 per person and $100,000 per occurrence, 21 years of age or older, a clean driving record, and ability to meet CCS vehicle safety requirements
Position status and expected schedule
- Full-time, 40 hours per week.
- Monday through Friday 8:30 am to 5:00 pm.
