Position provides harm reduction services to individuals at highest risk of acquiring HIV/AIDS, Hepatitis C/B, and other infections. Harm reduction services are designed to engage individuals who may not be ready to stop substance use by offering strategies to reduce risks. Services provided include Syringe Exchange, Overdose Prevention, client HIV, Hepatitis C, STI testing and support of clinical medical services. Position works closely with vulnerable and stigmatized populations: those experiencing mental illness, substance abuse disorders, deep trauma history, and sporadic engagement in medical care. This position requires the ability to work in an effective, respectful and culturally competent manner with clientele, community partners, and team members to provide a full complement of services that prevent infections and work to eliminate health disparities. This non-licensed position works under the medical authorization of the Deputy Health Officer. Position provides services in field based and clinic based settings with service hours scheduled to meet population needs. This position works a flexible schedule including evenings, nights and occasional holidays. Position requires the ability to read, write and speak in Spanish and English.
This position will be based at the Southeast Health Center and work will be performed in the field and other remote locations.
As the Community Health Specialist 2, you will:
  • Authentically engage with empathy, compassion and respect using a harm reduction approach - based on client readiness – when exploring and supporting behavior change to reduce substance use related harm
  • Collect, contain, transport and unload biohazardous waste, recycling and trash following program guidelines and procedures.
  • Accurately document and distribute Naloxone (prescription medication that reverses opioid overdose) per Oregon law and to manage and secure HIPAA documents
  • Ensure supplies are adequately prepared and stocked before after shift following program guidelines
  • Obtain testing consent, order labs and collect specimens (blood venipuncture / finger puncture, pharyngeal)
  • Perform rapid HIV / HCV tests
  • Review test result options with client (in-person, phone, My Chart and assist with My Chart registration as needed)
  • Proactively and competently engage men who have sex with men and people who inject drugs in health promotion counseling that is tailored to the client being served.
  • Make referrals to needed services based on assessed behavior change readiness
  • Adhere to program schedule and service delivery guidelines to ensure consistent operations in an effort to reduce staff and client frustration
  • Participate in team meetings, routinely review meeting minutes, and be fully responsible for project related assignments
  • Communicate professionally - phone, email, writing: Build and maintain cooperative working relationships; competently navigate EPIC, MMIS, PhTec and other electronic systems; comply with program, clinic, medical authorization, state, federal and grant guidelines/rules/laws; routinely use effective time management and self-management skills; comply with and promote improvement of all safety procedures
  • Work in a culturally competent manner with: diverse communities of color, people who inject drugs, LGBTQ+ community (Lesbian, Gay, Bisexual, Transgender, Queer), incarcerated individuals and individuals involved in criminal activity

Minimum Required Qualifications/Transferable Skills*:
Any combination of experience and training that would likely provide the knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be as noted below. All non-traditional education, training and experience will be evaluated.
  • Education/Training: Equivalent to an associate's degree AND
  • Experience: One (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position
Other:
  • This position requires the ability to speak, write and read fluently in the Spanish and English languages. The incumbent will provide direct service to highly vulnerable and stigmatized populations: (1) People who use drugs; (2) Individuals experiencing mental illness; (3) Men who have sex with men; (4) Culturally/ethnically diverse communities including the African American and Latinx communities.
  • Must have a valid State issued driver license
Preferred Qualifications/ Transferable Skills*: You do not need to have the following preferred qualifications/ transferable skills to qualify. However, keep in mind we may consider some or all of the following when identifying the most qualified candidates. Please clearly explain on your application how you meet any of the following preferred qualifications/transferable skills.
  • Previous training or experience with HIV, Hepatitis C, STI testing and/or phlebotomy
  • Previous training or experience providing syringe exchange services
  • Transferable skills: Your transferable skills are any skills you have gained through education, work experience (including the military) or life experiences that are relevant for this position. Be sure to describe any transferable skills on your application and clearly explain how they apply to this position.
