JOB TITLE: Habilitation Specialist II $500.00 sign on bonus
GENERAL STATEMENT OF DUTIES: Responsible for the learning experiences and supervision of persons with intellectual/developmental disabilities. Will demonstrate professional conduct to include compassion, dignity, and respect when interacting with other staff, family and people we serve and support.

ESSENTIAL JOB FUNCTIONS: The following is a summary of the essential functions of this job. The incumbent may perform other duties, both major and minor, that are not mentioned below, and essential functions may change from time to time.
- Ensure protection from abuse, neglect, mistreatment and exploitation in regard to the health, safety, comfort, and well-being for the people we support by adhering to all related policies.
- Ensure that people being served have the right to life, liberty, human security, dignity and are respected.
- Support the people being served with living a life that includes self-determination, choices, and promotes attainment of personal outcomes.
- Provide the necessary therapeutic assistance and/or training to people we serve in a variety of settings, per their staff action plan.
- Ensure the implementation of a person’s staff action plan utilizing as many appropriate resources in the person’s community as possible.
- Ensure daily service documentation requirements are met and that information is correct and complete prior to submission.
- Review and understand the IPOP’s for the people we support and report any change of information and/or concerns to the program coordinator.
- To arrange for transportation needed to successfully implement the Staff Action Plan by helping people access and use natural community supports (family and friends) and public transportation whenever possible.
- Provide mobility training for maximum independence to travel to and from work, and/or day habilitation sites, community stores, recreational sites, etc., such as traveling by public transportation.
- Assist in activities of daily living as may be necessary, i.e., toileting, eating and ambulating, including lifting assistance as needed.
- Report all incidents of federal, state and local law enforcement agencies presenting search warrants, subpoenas for information or testimony from agency personnel or any unannounced visitors to supervisor.
ACCEPTABLE EXPERIENCE AND TRAINING:
- High school diploma or GED
- Valid NYS drivers’ license with acceptable driving record.
