Job Summary“The Home Health Field RN is a nursing professional care provider under the direction of the director or designee who utilizes the nursing process, nursing standards and physicians orders to plan, provide and direct nursing practice. Accomplishment of nursing practice that includes practice elements as defined by the Kentucky Board of Nursing and in accordance with the Ephraim McDowell Health nursing professional practice model. Performance that accomplishes patient and family centered care as central to practice and a foundation of caring, compassion, competence, communication, collaboration and teamwork. Responsible to participate in shared nursing leadership to accomplish positive outcomes, education, respect, dignity and safety for the patient and their family. Accountable for leadership of self and others. Exhibits F.I.R.S.T. values (Friendliness, Innovation, Respect, Service and Trust).”
The Home Health Field RN performs hands-on nursing care as well as plans, organizes, and directs home care services. The Home Health Field RN is experienced in nursing with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of the individual and families within their homes and communities. -
Working Conditions, Hazards and Physical Effort
Works in a well-lighted, ventilated and air conditioned area when in the office. Schedule may consist of weekends, holidays and shift work. Excellent observation, verbal and written communication, and problem solving skills as well as basic math skill and nursing skills required. Visual acuity and hearing to perform duties.
Physical endurance for prolonged periods of standing, walking, stooping, heavy lifting, kneeling and bending are required. Risks exposure to disease, infection and physical strain and assault. Risks exposure to blood, body fluids, and tissue. Must be able to position and/or transfer patients as well as supplies and equipment. Must be able to move or position 50 lbs. independently.
Must be a licensed driver with an automobile that is insured in accordance with state and Organization requirements and is in good working order.

Minimum Qualifications or EquivalentsEducation: Graduate of an accredited ADN, BSN, MSN, or doctorate nursing program. BSN preferred.Experience: One (1) year home health experience and/or one to two (1-2) years of recent acute care experience in an institutional setting preferred.EQUIPMENT USED/SPECIAL SKILLS REQUIRED: (See Competency Checklist)

Licenses, Certifications, and/or RegistrationsCurrent RN license in the state of Kentucky. Basic Life Support certification (successful completion within three [3] months of employment). Valid driver’s license.
