General Function:

The primary focus of this position is to oversee and/or provide case management supports specifically related to housing and employment stability for all eligible VOAWW participants. This position will report directly to the Senior Director of Personal Support Services and will work closely with each VOAWW department Director and FCS staff. This position will work closely with City, County and State officials, as well as develop partnerships with community members. This positions primary focus will be to oversee agency expansion efforts for housing and employment stability, program billing, database accuracy and monitoring program success.

Principal Activities:

  • Participating in and leading strategic conversations.
  • Networking with City, County, and State officials, as well as community members.
  • Trains VOAWW FCS staff on FCS service delivery and policies.
  • Conducting meetings and presentations that bring awareness of VOAWWs FCS program to other VOAWW Departments and community entities.
  • Develop plans for future program growth in collaboration with the DCOO, PSS Senior Director and other VOAWW Directors.
  • Maintain agency FCS databases. Regularly monitors and tracks expiring assessments and notifies Amerigroup/VOAWW staff accordingly.
  • Monitor and track FCS billing and remittances.
  • May need to meet directly with individuals in need of resources and referrals, either by phone or in-person and provide follow-up services. This position may be responsible for a small caseload and will collaboratively develop a plan to connect the person with needed resources.
  • Develop expertise in the Supported Housing and Employment arena by working with local housing authorities, providers, partners, and additional resources regarding affordable housing and job availability, subsidies, voucher capacities, and supportive wrap-around services.
  • May input client data and information into the Extended Reach database.
  • Cultivates, develops, and retains positive relationships within the community including with employers and landlords.
  • Keeps current with best practices as related to trauma-informed care, FCS requirements, community support and poverty.
  • Strictly follows HIPAA guidelines and VOAWW policies.
  • Other duties as assigned, are related to the general functions of client support outlined above.

Qualifications:

  • A College degree in social work, human services, or a related field is strongly preferred. Related experience can be substituted for educational requirements.
  • Experience and/or familiarity with Foundational Community Support Services is strongly preferred.
  • Valid drivers license and reliable transportation.
  • Proficiency in public presentation skills along with the ability to be a positive and effective representative of VOAWW to all community partners, legislators, officials, and other audiences.
  • Effective interpersonal, written, and communication skills.
  • General knowledge of Microsoft Suite programs.
  • Experience and ability to influence and collaborate with a variety of personalities and diverse backgrounds.
  • Ability to work independently and in a team. Must be self-motivated, with the ability to be flexible in a constantly changing environment.
  • Must demonstrate effective attention to detail, and the ability to comply with project and work deadlines.
  • Ability to pass and maintain an acceptable background check record per State guidelines

Physical Requirements:

  • Requires a combination of periods sitting at a desk and working on a computer, and periods of continuous walking, standing, bending, stooping
  • Must be able to lift up to 30 pounds at times
