About the Organization:
The Independent Living Resource Center (ILRC) is a disability focused non-profit providing services and advocacy to those experiencing all disabilities, of all ages, and all income levels. Our mission is to promote independent living and full access for individuals with disabilities through advocacy, education and action in our communities. The organization has been based in Santa Barbara since it opened in 1976 and serves the Central Coast Counties of; Ventura, Santa Barbara, and San Luis Obispo. Services are provided by utilizing a team approach where the consumer is an active member in their goal attainment. Services provided by the organization include; Housing, Personal Assistance, Interpreting, Peer Support, Assistive Technology, Youth Transition, Older Individuals who are Blind Program, Advocacy, and Emergency Preparedness. ILRC is actively engaged in all of our counties.
About the Ventura County Aging and Disability Resource Center (VCADRC):
In 2015, ILRC and the Ventura County Area Agency on Aging (VCAAA) came together to represent the core partnership in the establishment of the VCADRC which also includes extended partners representing various sectors and community resources. The VCADRC’s purpose is to provide a single more coordinated system for people seeking reliable information and access to Long-Term Services and Supports (LTSS) therefore bridging the gap between services for people with disabilities and older adults. This purpose is achieved by building community partnerships, providing services using a person-centered approach, and reducing the number of barriers for accessing services. There are ADRCs nationwide and they are intended to act as a “No Wrong Door” system which enables people of all ages, incomes, and disabilities to connect with any one ADRC partner organization for accessing a wide array of Long-Term Services and Support options in the community. ADRCs support the needs of caregivers and respect the diversity of families and cultures that make up their local communities.
Responsibilities:
- Develop a comprehensive community outreach plan to inform the public about the Ventura County ADRC; track outreach contacts, service hours, and results;
- Conduct consumer intakes, needs assessments, and follow-up procedures;
- Manage all referrals and refer to appropriate staff/agencies using the no wrong door policy
- Draft written Independent Living Plan goals with all consumers or obtain waiver and monitor, follow-up on consumers' progress and document outcomes;
- Create consumer files and document all services as provided and update on a timely basis;
- Advocate with consumers when access rights are limited or denied;
- Compile and submit consumer and service data;
- Refer consumers to resources and support programs that will enhance their independence;
- Facilitate the maintenance of housing resource information for referral to consumers and assist consumers in their application process to available housing programs;
- Provide independent living skills training (ILS) to enable consumers to achieve housing, transportation, in-home care, disaster preparedness, and self-advocacy goals.
- Advocate with consumers when access rights are limited or denied.
- Recruit, interview, and check references of potential Personal Assistants (PAs) for referral to consumers and maintain a list of available PAs.
- Refer no less than 3 prescreened PAs to consumers per request.
- Serve on Committees to promote Systems Advocacy in the areas of housing, transportation, and Personal Assistance and keep in the know of area issues.
- Provide information and assistance, options counseling, and short-term service coordination to all consumers receiving services
- Utilize and follow ILRC policy, procedure, and service provision manuals;
- Participate fully as a member of a working team and promote harmonious working relationships
- Participate in monthly ADRC meetings
- Document all services provided and enter them in CIL Suite on a timely basis
- Conduct agency business in a professional manner
- Report suspected abuse or concerns to appropriate authorities as a
mandated reporter
- Other duties as assigned
Essential Qualifications:
- Familiarity with human services programs affecting persons w/disabilities of all ages
- Motivating interest in disability/independent living field
- Excellent written and verbal communication skills
- Experience working with persons with disabilities and older adults with emphasis in the areas of advocacy, housing assistance, and community access issues
- Ability to implement training various service delivery models;
- Ability to meet deadlines
- Computer skills including data, social media, and research
- Reliable personal vehicle for work travel
- Must maintain valid driver’s license and a satisfactory driving record
- Ethical work practices: honest, dependable, team-oriented
Preferred Qualifications:
- Personal experience with disability
- Bilingual English/Spanish or English/ASL
