Job Summary
This position will be responsible for the activation of relationships, internal and external to the organization, that will provide the foundation necessary deliver on the population health strategy, further reinforcing Priority Health's commitment to improved member outcomes. The role will have three main functions. 1) The role will support the optimization of value-based care, while working intricately with our two system delivery partners. 2) The role will develop and lead our person and family engagement (PFE) approach across all lines of business. And 3) The role will engage with community-based organizations and partners, further reinforcing our commitment to community.

As this is a new function, the role involves building, developing and managing a high-performing team.

The Director of Population Activation reports directly to the VP, Population Health and Health Equity, within the government markets division. This role will regularly interact with the Director of Quality, Director of HEDIS and Data Analytics, and the Director of Value Based Programs. The role will also engage regularly with the customer experience team.
Essential Functions
  • Leads member support: oversees all non-marketing related efforts to activate members from Medicare, Medicaid, and Individual markets- inclusive of oversight of community health workers, navigators, and wellness coordinators
  • Leads community support: develops and maintains key relationships across the state, for all lines of buisness, that will directly benefit person and family activation.
  • Leads delivery support: acts as a liaison to the provider strategy and incentive teams as they support value- based relationships
  • PFE: Acts as a subject matter expert on Person and Family Engagement
  • Oversees special population teams
Qualifications
  • Bachelor's Degree nursing, public health, health care management, or other related degree -Required
  • Preferred Master's Degree MPH or MBA
  • 7 years of relevant experience Strategic leader with demonstrated ability to collaborate with multiple business functions and drive change Preferred
  • Healthcare experience - Preferred
