Hospice of Lenawee is hiring for a:

  • Director of Patient Care RN - Full-time - Hospice*

Education/Credentials of a Director of Patient Care:

  • Graduate of approved school of nursing.
  • Registered Nurse with a valid Michigan license.
  • BSN, preferred.
  • Hospice certification, preferred.
  • Valid driver license and vehicle available with current auto insurance.

Essential Job Responsibilities of the Director of Patient Care:

  • Collaborates with leadership to carry out the organization’s goals and strategy related to patient care.
  • Plans, leads, develops, coordinates, and implements policies, processes, and training initiatives to support the organization’s patient care needs.
  • Monitors state, federal and accreditation regulations and standards and assist in educating staff on changes in the regulations.
  • Coordinates development and maintenance of clinical objectives, standards of practice, policy and procedure manuals and job descriptions.
  • Participates as a member of the Medical and QAPI Committees.
  • Establishes, implements, and evaluates HOL’s care, treatment, and services to comply with federal, state, local and CHAP regulations.
  • Assists in the review of the policies and procedures, forms, and protocols.
  • Coordinates proposed annual patient care program budget for President and CEO review and Board approval.
  • Promotes financial stewardship and adjusts staffing model as needed to remain within budget expectations.
  • Oversees accreditation process including CHAP and CMS Licensing.
  • Participates in community outreach/education and census growth initiatives and coordinates efforts with Community Outreach Coordinator, Clinical Leadership, and Direct Care Staff.
  • Promotes a culture of philanthropy with internal and external stakeholders.

Qualifications of the Director of Patient Care:
Required Knowledge, Skills, and Abilities of the Director of Patient Care:

  • Demonstrates ability to identify complex problems and review related information to develop and evaluate options and implement solutions.
  • Demonstrates exceptional time management, organizational, interpersonal, and networking skills.
  • Possesses knowledge of business and management principles involved in strategic planning, resource allocation, leadership technique, and coordination of people and resources.
  • Demonstrates excellent verbal and written communication skills.
  • Consistently demonstrates through action and attitude a commitment to establishing and maintaining positive relations with patients, families, physicians, team members, vendors, and others.
  • Demonstrates an understanding of the interdisciplinary team approach and the ability to facilitate its function.
  • Demonstrates ability to mentor patient care staff as independent practitioners in the home care setting.
  • Possesses knowledge of community health organizations, practices, and resources.
  • Demonstrates ability to make sound clinical judgments and act upon them in a timely manner.
  • Demonstrates ability to plan, assess, implement, and evaluate nursing care for Hospice of Lenawee patients and families.
  • Demonstrates ability to supervise and direct care given by others.
  • Demonstrates leadership and management skills.

Experience of a Director of Patient Care:

  • Two years nursing experience, required.
  • Prior experience in a health care environment, home care or hospice preferred.
  • Experience in a leadership and supervisory position, required.
  • Equivalent combination of education and experience may be substituted for specific requirements.
  • Experience with dying patients and their families, hospice experience preferred.
  • Experience in health care, required.

Position Status/Classification: Exempt, Regular, Salary, Patient Care, Management. Stipend given for Holiday, Weekly and Supervisory On-Call duties. Compensated at Hourly wage rate, when working as Primary Nurse
