Director of Integrated Supportive Services
Job description
The Director of Integrated Supportive Services is responsible for being the champion of good patient health outcomes and integration of supportive services into medical care. The Director supports health outcomes and integration of services through oversight of the behavioral health team, Prevention Services Supervisor and RW Case Management Supervisor. Responsible for program development, planning, implementation, quality assurance and evaluation of medical case management programs; assists with policy development and implementation, developing and maintaining program budgets and grant management for Ryan White and HOPWA programs including grant writing and required reporting.
Responsible for ensuring cross departmental collaboration and service integration that promotes positive patient health outcomes and positive patient experiences. Responsible for ensuring behavioral health integration into primary care; program development, planning, implementation, quality assurance and evaluation within the behavioral health program. Investigates grievances filed by patients, community members, and/or other staff members against behavioral health services staff. Coordinates training of all behavioral health services staff; works with Data and Quality Team to ensure data integrity, monitor program quality and complete required reporting.
Provides supervision for the HIV Case Management Supervisor, the Prevention Services Supervisor, the Counselor, Behavioral Health Consultant, Behavioral Health Case Manager and Patient Care Coordinator. Works with Leadership Team to monitor organizational outcomes and to strategize collaboratively on operational enhancements to improve outcomes.
Requirements
- Master's in social work or counseling preferred.
- Clinical licensure preferred.
- Social work public health, and/or counseling required.
- Experience in leadership and supervision required.
- Ability to give constructive criticism.
- Strong verbal and written communication skills.
