Responsibilities:
Director of Care Navigation Responsibilities:
  • Work in conjunction with Regional and Branch leadership to enforce the ideals of the Care Transitions Model with emphasis on patient experience, patient education, referral source partnerships, and outcomes data discussions. Develop and maintain an environment that supports the Care Transition Nurses and Home Care Navigators by managing, organizing and facilitating key reporting duties including, but not limited to: providing effective navigation services, achieving quality standards, communicating work flow processes, ensuring integration of a clinical team approach and conducting personal performance review for each direct report.
  • Lead and develop team members, including onboarding and training. Evaluating effectiveness of patient navigation by measuring, reporting, and maintaining essential data/databases on established benchmarks and metrics. Manage integrity and quality of data for customer reporting by identifying negative trends and collaborating with clinical and operations leadership to implement Action Plans to meet customer expectations and needs; communicating and following up on action plans in a timely manner to meet customer requirements for data/reporting; drive process improvement; and develop and implement a standardized reporting package to demonstrate program outcomes and support performance improvement activities.
  • Track the performance of the clinical programs and ensure they are in line with Company goals (growth, quality, practice guidelines, etc.) and identify opportunities to streamline care practices in care transitions and navigation. Develop and implement systematic, enterprise-wide patient navigation workflow processes, protocols, policies, and procedures.
  • Collaborate with Director of Market Development to identify market targets and provide clinical liaison/field support to meet Company growth metrics and standards.
  • Serve as the Intermediary between customer and branch to ensure essential functions of patient navigation are met.
  • Serve as a resource for problem/conflict resolution as issues related to patient navigation as identified.
Qualifications:
Director of Care Navigation Qualifications:
  • High school diploma. Bachelor’s degree in Business Administration or Healthcare Management preferred (equivalent experience base may be substituted for Bachelor’s degree)
  • RN license required
  • 5 or more years of experience
  • Comprehensive knowledge of transitional care theories, concepts and practices and ability to use in complex, difficult and/or unprecedented situations
