A new opening for a Director of Care Coordination has become available at a highly accredited, and respected acute care facility outside of Fremont, California. This 160+ bed hospital is staffed with 500+ employees who are highly trained in their specialty.
The responsibilities of the Director of Care Coordination will vary, but they will have 20+ reporting up to them, while reporting directly up to the Executive Director of Continuum. This is a large role, as the Director must be comfortable with directing, coordinating, and controlling all aspects of the operating functions, processes, and staff members of the facility. This role is in charge of patient and employee to oversee the entire facility in ensuring the safety and satisfaction of every person that walks through the door. In the case of absence of the Director, one must appoint another staff member to take their place and hold all responsibility until their return. The Director of Care Management may carry responsibilities including:
- Coordinating meetings with staff members and management team to implement changes that will help to achieve all improvement targets
- Take part in interviewing, hiring, and performance reviews within the facility while managing the daily duties of population health department
- Focus on improving all healthcare outcomes while monitoring performance standards
Hospital is looking for a minimum of 8 years in clinical and managerial roles within a multi-faced healthcare system. A current CA nursing license is highly preferred but not required at this time.
