Job Description:

- Assist with shopping needs by transporting individual to shop with Member.

- Cleaning in individuals home

Schedule: Saturday 11:00AM - 3:00PM

Wage: 15.75 Per Hour /.58 Per Mile while driving member.

Additional Requirements

- Current Driver’s License and Insurance required.

If hired, Consumer Direct Care Network (CDCN) will not be your legal employer on record. If hired by a member or their Managing Party, your legal employer on record would be the Member. CDCN's role is to assist the Member or Managing Party in recruiting a Direct Care Worker. The Member or Managing
Party has the sole authority to hire or dismiss any potential Direct Care hired, Consumer Direct Care
Network (CDCN) will not be your legal employer on record.

Pay: $15.75 per hour

• Day shift
• No nights
• Weekend availability
• Weekends only

• South Range, WI: Reliably commute or planning to relocate before starting work (Required)
