Job description
The role of the DIRECT CARE/MENTAL HEALTH WORKER for Residential Programs is also known as a Mental Health Worker for Residential Programs.

Department: Long Term Structured Residence (LTSR)

Reports to: Nurse Manager and Director of LTSR

Classification: Full Time, Part-Time, * per diem ; non-exempt

  • This listing is for a per diem Direct Care/Mental Health Worker, minimum guaranteed 8 hours per week in accordance with program needs.
Qualifications:
Minimum high school diploma or equivalency; and

6 months of formal training in mental health field, or a related field; and

Valid Driver’s License.

Responsibilities:
Provide training and assistance in activities of daily living and related self-care skills. Areas include bathing, toileting, personal hygiene, grooming, dressing, care of clothing and shaving.

Provide training and assistance in social skill building, vocational training and employment, community inclusion, and health and wellness. Areas of focus include leisure planning, transportation, meal preparation, housekeeping, shopping, scheduling appointments, fiscal management, etc..

Assist individuals in working towards the completion of their recovery goals as written on their treatment plan.

Participate in multi-disciplinary treatment team meetings as assigned.

Encourage the development of an on-site peer supported government.

Accompany individuals in recovery into the community to medical appointments, mental health court commitment hearings, family visits and other appointments.

Assist with meal preparation and clean up as needed. Follows individual dietary restrictions/special diets during mealtimes as prescribed.

Complete resident “rounds” as assigned to ensure that all individuals in recovery are safe and in their expected locations.

Provide a comprehensive shift report at shift change to ensure that all meaningful information is passed on for proper continuity of care of individuals in recovery.

Assist with the general upkeep of the facility and participates in light cleaning and household chores.

Timely reporting of all crises and unusual incidents to Shift Supervisor, Nurse Manager and/or LTSR Director (Immediately if indicated; otherwise within 24 hours).

Complete detailed documentation for individuals in their clinical record and in other areas as directed. Such documentation should be timely, legible, and provide an accurate depiction of progress, challenges, medical status, etc.. Satisfies compliance with regulations regarding record keeping and reporting.

Participate in Quality Improvement Program as assigned.

Attend all meetings and training sessions as assigned.

Maintain compliance with all Creative Health policies and procedures with no unauthorized exception.

Other duties as assigned.
