You will support our residents and work hand-in-hand with our already wonderful team of compassionate and enthusiastic caregivers. The position is responsible for the coordination of all programming and staffing aspects of the Memory Care Program at Meadowbrook while adhering to state policies governing dementia resident care

Your Job

• Treat and care for residents and their property with dignity and respect
• Offers support in the creation and presentation of the quarterly wellness plans
• Assists the residents and their families in maintaining the physical and emotional health of the individual
• Provide continuous restorative nursing in conjunction with the resident’s activities of daily living
• Coordinates all aspects of life for Memory Care residents including evaluation of residents upon admission and development of personal plan or program
• Attend various in-service training meetings to maintain skills
• Coordinate nursing services with other resident services to ensure continuity of the resident’s total regimen of care

Who You Are

• Currently licensed to work as a LPN or RN in the state of IL.
• A professional team-player with a commitment to maintain a reliable shift history
• Knowledge of Dementia care is preffered but not required
• Ability to relate in a professional and positive manner with all community staff and the community at large
