Basic Job Purpose/Function

Under the clinical supervision of the Director the Crisis Case Manager provides Behavioral Health services in both the community, in the clinic, residents homes, hospital settings and other areas as needed. The Crisis Case Manager works as a member of the crisis team, collaborating with agency professionals through case planning and service coordination.

This position is a valued member of the team who works with individuals, families and community partners to meet client needs such as stable housing, access to health care, substance abuse treatment, stable transportation and emphasizes crisis stabilization, resilience and recovery. This position works independently as well as collaboratively, uses sound judgment, conforms to adopted policies and procedures, solves problems and achieves results consistent with expected goals and objectives.

Essential Job Duties

  • Works with the Director in the implementation of departmental programs and procedures to accomplish local, state and federal Behavioral Health Department goals and policies.
  • Provides transportation, outreach, case management and health education services to all assigned clients. Services are provided in a variety of community settings, with consideration to client preference and department need.
  • Actively participates as a member of the team in joint case planning and coordination with internal multi-disciplinary team and agency providers.
  • May provide payee services or support clients to establish independent payeeship.
  • Utilizes agency clinical software systems to prepare current client records and reports as necessary and required. Maintain timely and cogent progress notes and plans. Maintains proper and accurate documentation of all client and community encounters.
  • Ensure that client records and other documentation are up-to-date and justify the case management services.
  • Participate in meetings as required. Attends local, regional, and/or state meetings as requested by the Director/DCR.
  • Within the scope of the position, perform duties independently or during crisis work with a colleague to provide transport for an appointment, as a part of a mobile crisis response or to support the client by transporting them to their place of residence.
  • This position’s work in the community projects and promotes a positive image of the department and county.
  • Provides coordination between behavioral health integrated programs, strategic community partners, public health and team clinicians/crisis providers.

Requirements/Minimum Qualifications

Education: Bachelor’s Degree in social work or human service-related field and prior internship/employment in human services.

Experience: Experience working with the severe and persistent mentally ill and or with the disabled population is preferred.

  • Knowledge of all state and county requirements contained in the Revised Code of Washington (RCW) and Washington Administrative Code (WAC) or other directives to meet quality and record keeping requirements.
  • Knowledge and adherence to HIPAA/FERPA and CFR 42 Part 2 as it specifically pertains to all behavioral health programs.
  • Knowledge of effective organization and prioritization of multiple program functions.
  • Skill in following the overall strategy for the provision and allocation of behavioral health services.
  • Skill in coping strategies to manage patient and documentation load effectively.
  • Skill in record keeping, organization and prioritization functions.
  • Ability to work with multicultural, multi-language and disenfranchised populations.
  • Ability to solve problems using sound professional judgment.
  • Ability to assure prompt response to behavioral health emergencies with all necessary county departments and state and local agencies as directed by the Director/DCR.
  • Ability to communicate effectively verbally and in writing with patients, staff and the public.
  • Ability to maintain continuing liaison and professional working relationships with behavioral health agencies and other medical and human service-related agencies and providers.

Licenses, Certificates and Other Requirements

  • Submit to a criminal background check, as required by Washington State Law.
  • Washington State Driver’s License or ability to be a licensed driver in Washington State.
  • Documented completion of continuing education on an annual basis to maintain certification and licensure as applicable.
