Overview:
The Clinical Admission Coordinator (nurse degree/licensure required) is responsible for the clinical assessment aspects of the intake process for all skilled and long term care nursing referrals. The Coordinator must demonstrate clinical competence in patient assessment and excellent communication skills with referring sources, physicians, case managers, patients, families, and insurance companies. Organization skills and attention to detail are required for completion of the assessment. Knowledge of skilled criteria is necessary to determine appropriate level of care. The position supports regional marketing efforts and promotes the Lutheran SeniorLife continuum of care system.
The home office for this position is located in Butler, PA. This position will require the incumbent to travel to Armstrong Hosiptal and surrounding areas.

Qualifications:
- Nursing School Diploma and license from Commonwealth of PA required. Licensed as a RN, LPN by the Commonwealth of Pennsylvania. In lieu of licensure, candidate must have demonstrated 5 years of related case management experience in an applicable health care setting.
- Requires 2 -3 years of strong clinical experience in geriatric and/or adult setting.
- Demonstrates understanding of skilled and long term care, Medicare and managed care as it relates to the CMI.
- Must possess knowledge to assimilate information and communicate effectively to physicians, social workers, internal staff and families in a professional and timely manner.
