Provide clerical/non-clinical support for Conduit Health Partners business areas. Role includes coordination and communication that supports patient logistics, scheduling and care coordination related to books of business. Possesses strong communication and interpersonal skills in addition to an understanding of medical terminology. The role requires team participation and willingness to accept delegation. Flexibility, autonomy, organization and the ability to multi-task are essential characteristics.
Essential Job Functions
Facilitates transportation coordination requesting service type based upon information provided by provider, RN Transfer Specialist dependent upon assignment.
Facilitates scheduling and subsequent patient communication if applicable to assignment.
Facilitates follow up communication with providers/ care team at facilities to maintain awareness of case status. This includes; but not limited to bed assignment status, transportation arrangements, inquiries related to potential for readmission, pending e-visit as well as needs for related follow up.
Accurately enters and retrieves information within the EMR as well as other databases (ADT, scheduling) while ensuring compliance to policy and regulatory processes and data collection.
Maintains professional collaborative relationships with members of the care team.
Participates in process improvement, professional development, peer development, peer review and shared governance.
Responsible for other tasks and analysis as requested /directed.
Education Qualifications -
Required Minimum Education:
Diploma
High school or equivalent
Preferred Education
Associates Degree
Health or informatics related
Minimum Qualifications
Other Knowledge, Skills and Abilities Required
Customer service, medical terminology
Other Knowledge, Skills and Abilities Preferred
Office administration, EPIC
