Sana Lake Recovery; is a uniquely different addiction and mental health treatment center from the vast majority of centers in the country as we solely focus on delivering better outcomes for our members measured in terms of quality of life. Our system of care models the addiction programs for physicians and airline pilots which have achieved materially better outcomes than the typical treatment center over a long period of time. Our staff deliver highly personalized care using dynamic treatment and recovery plans managed by specialized care coordinators. Treatment is holistic evidence-based across a full continuum of care, from acute to chronic care programming with extended care support for five years or more. Programming is delivered by professionally credentialed medical, therapeutic, clinical and peer specialist staff. Frequent assessments are conducted of the members' progress of achieving goals they can reasonably attain to adjust the treatment plan as needed. The staff are highly trained and credentialed professionals that care about the member finding recovery for life in everything we do.
The Community Support specialist is responsible for finding and linking members with internal and external resources as needed to meet needs. They will assist members in managing outside stressors (legal, financial, vocational, etc.) according to the best interests of the member's progress in recovery. The Community Support Specialist will collaborate with the Treatment Team to formulate the Master Treatment Plan and assist in coordinating discharge planning. They will be responsible for maintaining communication between treatment team members, family, referral sources and other service providers as needed.
POSITION REQUIREMENTS/QUALIFICATIONS
EDUCATION:
Bachelor's degree, associate's degree
EXPERIENCE:
Required experience depends on level of education. Minimum 1 year of experience in substance use treatment experience and case management
LICENSURE/CERTIFICATION:
- Completion of CSS 101 training.
- CPR and First Aid Certified, preferred
- CPI Certified, preferred
SPECIFIC AREAS OF RESPONSIBILITY TO POSITION
- Complete a wellness evaluation in order to gather information related to areas of legal, vocation, nutrition, and education, etc.
- Screen/assess for self-harm/risk factors and create safety plans with members as needed. Share information with therapist or counselor
- Participate effectively in multidisciplinary treatment plan reviews, demonstrating adequate preparation and understanding of member issues, progress, strengths and weaknesses, areas of resistance, positive and negative strategies, and use good clinical judgment in moving member through treatment levels.
- Develop interventions that will accomplish goals and objectives and state the planned frequency of interventions
- Maintain clinical records according to program policies and those of licensing and accrediting agencies, document the member's progress accurately as it relates to treatment goals and objectives, ensure that documentation is timely, consistent, clear and articulate and keep clinical records secure at all times
- Continue treatment planning process throughout member's stay in treatment, opening new problems and developing or revising treatment plans as necessary
- Complete and/or review activity and group plans in advance for the Clinical Director including supplies utilized and/or needed
- Conduct educational groups (lectures/videos/discussions) effectively
- Address problem behaviors, confront counter-productive behavior appropriately in a manner that elicits respect and bonds with the member while still enforcing program rules and regulations in a firm but fair manner
- Assist members to understand treatment goals and give them a clear picture of where they stand with regard to progress toward recovery as seen by the therapist and the multidisciplinary treatment team
- Maintain healthy boundaries
- Understand and apply the program's clinical philosophy
- Provide individual sessions to members at least one (1) time per week
- Be effective in finding and linking member with internal and external resources
- Prepare to maintain recovery post-discharge and help develop a discharge plan that meets member needs for ongoing support
- Assist member in developing transition plan
Availability
At least 1 Saturday and 1 Sunday a month
1 late night a week
