The Community Outreach Manager – Contracts & Grants is responsible for driving partner engagement and project management efforts at Health Net of West Michigan. This position leads and manages special projects, coalition/relationship building, and contract management processes to support the screening, outreach, & navigation functions of the organization’s core work. Supervises the Project Managers and co-facilitates Community Outreach (COR) team meetings. Responsible for understanding grants, contracts, and project scopes and tracking key deliverables, convening partners, drafting & submitting reports, troubleshooting issues, and driving project goals to completion. This position delegates project assignments to staff, utilizes project tools such as Monday.com & Microsoft Office 365 to track relevant deadlines and tasks. Leads & delegates M-SHIELD, Priority Health, Healthy Aging, WK Kellogg, and additional organizational projects assigned. This position functions as a member of a cross departmental quality assurance team that meets bi-monthly ensure organizational alignment across core work areas (screening, navigation, and outreach) to provide services to individuals/families by addressing health related social needs to achieve health equity. Community Outreach Manager reports directly to the Chief Innovation Officer.
The Community Outreach Manager will represent the organization as a key contact person for external agencies. They will work to nurture positive community, state-wide, and county-wide collaborative relationships to promote Health Net’s services in the broader community. The position specifically requires the ability to articulate Health Net’s services proficiently via written and verbal methods to communicate the broad scope impact of the work. The Community Outreach Manager position requires the ability to translate concepts into action and build meaningful relationships with persons from diverse cultural, social, economic, and ethnic backgrounds. Pay range: $55,000 - $65,000 per year.
Essential Job Duties and Responsibilities:
Leadership and Management
- Lead, supervise & manage the Project Managers and provide professional guidance & feedback
- Support the professional development of the Project Managers as needed
- Conduct & co-facilitate COR team meetings
- Track & assign project management tasks using Monday.com and other organizational tools
- Maintain relevant logic models, evaluation plans, operational plans, SOW’s, and workflows related to project/contract scope
- Provide quarterly summaries of project/grant activities to leadership team
- Serve as a mentor or coach to direct reports as it relates to their professional development goals
- Evaluate employee performance through the review of completed work assignments and departmental metrics
Writing | Grant & Project Coordination
- Coordinate grant writing efforts and complete applications as assigned
- Manage grant project documents and information
- Update and track implementation plan progress
- Maintain internal reporting calendar in collaboration with the Data & Reporting Manager to ensure that all reporting is completed/submitted in a timely fashion
- Coordinate and carry-out logistics for meetings, events, and relevant activities for projects/workgroups/coalition activities
- Prepare and interpret reports/data results to community partners regarding Health Net services and programs
- Complete on-going assessment of program operations, and make recommendations for improvement, as needed.
Internal/External Communication | Relationship Building
- Timely dissemination of all relevant documentation to our partner sites
- Attend/facilitate relevant project meetings & convenings
- Work closely with key stakeholders to improve and expand community partnerships
- Coordinate agency and community wide partner alignment efforts
- Build relationships & network with civic organizations, social clubs, businesses, faith groups, and other community organizations that center on the issue of collective community impact.
- Participate in advocacy for improved access and quality of healthcare for low-income and un-(or under-) insured by attending selected governmental agency meetings, legislative sessions/meetings, and community organization meetings.
Education, Skills and/or Experience:
- Bachelor’s degree in health, human services or related field required
- Master’s degree in a field relevant to assigned areas is preferred (public health, public administration, other healthcare field).
- Experience in collective impact work, or fields relevant to assigned responsibilities.
- Time management and ability to multitask and navigate the field effectively both within updated technological systems and community development models
- Ability to problem solve and creatively offer best practice solutions to complex issues.
- Strong analytical ability necessary with a focus on quality improvement practices.
- Proficient computer skills in Microsoft Office computer applications and program software.
- Ability to work with multiple deadlines producing exceptional results.
