Mill Pond is a program operated by Lakeview Care Partners, Inc., in a rural community in Westfield, WI. Mill Pond is a CBRF that specializes in assisting individuals with Traumatic Brain Injuries, AODA, Physical and/or Developmental Disabilities, and other health conditions. Our goal is to assist these individuals tolearn the skills necessary to gain back their independence, with the hope that they will be able to live on their own with little to no assistance. Lakeview is looking for you! We are looking for a Community Integration Specialist in our Millpond location.

Purpose:
Provides direct care and treatment, supervision, and training for all program participants to meet company and regulatory standards of clinical services, care, risk management, and customer satisfaction. Assists clinicians in the planning, implementation of treatment and the program participant's day-to-day needs.

Key Responsibilities:
1. Functions at all times as a therapeutic agent by demonstrating teaching, reinforcing, anticipating, interacting, and noticing program participant behavior.

2. Tends to the program participant immediate needs for safety and performs all aspects of participant care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

3. Provides treatment in accordance with unit and facility policies through active participation with residents during activities, ADL's, school, work, meals, groups, etc., in a way that fosters independent functioning on the part of the resident.

4. Displays age appropriate interaction skills. Actively and professionally participates in exchange of information. Interacts with family members and others in professional manner.

Qualifications

1. Skill, Knowledge and Abilities: Knowledge of the sequela of brain injury and other neurological disorders. Knowledge of physical, cognitive, psychological, emotional, and/or communication disabilities. Ability to obtain and maintain Mandt certification. First Aid course required.

2. Education: Required: High school diploma or equivalent may include completion of certified nursing assistant course with certificate. Preferred: Bachelor’s degree in the area of Social Services. Certified as CNA if CNA.

3. Licensure: Valid driver’s license and good driving record preferred

4. Experience: Preferred one year experience working within residential based or behavioral program.

Job Types: Full-time, Part-time

Pay: $14.00 - $28.00 per hour
