WellAWARE/CVHS is seeking full-time Clinical Health Workers. CHWs will serve as front-line workers who live in the same communities that they serve. Sharing some lived experiences with clients will aid CHWs in building trust and vital relationships. CHWs will provide personalized assistance to improve clients’ access to care through strategies like transportation assistance, medical home connection, chronic health education, referrals to mental health resources, accompanying clients to appointments, assisting with forms and applications, advocating for clients’ needs, and generally navigating the complex healthcare system.We are seeking energetic, open-minded, and compassionate candidates to join our team. Must have a High School Diploma or GED and be fully vaccinated for COVID-19.
