Job Summary

The Community Health Worker Specialist (CHW) will provide outreach, brief coaching, and health related social support for children primarily ages five through eighteen and their caregivers. This position will focus on reducing barriers to accessing mental/behavioral health services through social services support, providing transportation to appointments, and coordinating care when navigating the primary care and mental/behavioral health system.


This role may provide basic education on mental health and wellbeing, brief coaching and discreet skill building for children, youth and caregivers, and coordination in accessing support to address identified needs. Further, the CHW will foster collaborative relationships with other providers and social support, specifically school staff, to ensure ease in coordination of care across service domains.


The CHW is a grant funded position (January 2023 thru December 2024) through the Washington State Health Care Authority. Funding may be extended by the Health Care Authority for an additional two years. Position is eligible for benefits. However, due to the nature of the position being grant funded, the pay rate will not change unless dictated by HCA’s contract.


Job Specific Duties and Responsibilities

  • Provide services to children ages five through eighteen and their caregivers focused on reducing barriers to accessing primary care and mental/behavioral health services through support and coordinated care when navigating the mental/behavioral health system.
  • Provide basic education on primary care and mental health and wellbeing, brief coaching and discreet skill building for children, youth and caregivers, and coordination in accessing community supports to address identified needs.
  • Further, CHWs will foster collaborative relationships with other providers and social support, specifically school staff, to ensure ease in coordination of care across service domains.
  • Promoting healthy development through support in navigating primary care for young children and families as well as connection to necessary resources and services.
  • Must have the ability to drive and care for patients off-campus and meet the driver qualifications per policy.
  • May offer additional support and services within the scope of their job description and role.
  • Performs special projects and other related duties as assigned.

Required Education and Experience

  • Lived experience with youth within the Grays Harbor Community.

Required Licenses, Certifications and/or Registrations

  • K12 Mental Health Community Health Worker Training required within 90 days of hire.
  • Valid WA driver's license with a satisfactory driving record; No more than two (2) moving violations within the past two (2) years.
  • Defensive Driver Course specified by SPMC required within 6-months of hire.
  • Drivers must be a minimum of 21 years of age.

Required Knowledge, Skills, Abilities

  • Strong organizational skills: ability to prioritize, and reprioritize, work as needed to meet objectives.
  • Professional and effective written and verbal communication skills. Ability to identify and employ communication strategies appropriate to the audience. Grammar and punctuation skills as needed to compose professional documents.
  • Proficient in the use of current technology and software, including intermediate to advanced Microsoft Office skills (Word, Excel, Outlook).
  • Proven ability to handle confidential information with discretion. Ability to develop and maintain accurate documentation and records.
  • Demonstrated teamwork and collaboration skills. Strong customer service and interpersonal skills; ability to maintain composure and interact calmly and professionally with co-workers, providers, patients and visitors in a fast-paced, high-stress environment.
  • Ability to work independently using good judgment, problem-solving, organization, and prioritization skills to complete assignments in a timely, accurate, and effective manner.

This is a temporary position with an annual base income of $62,000.
