Community-Health Workers (CHWs) provide social support, navigation and advocacy to HIV positive high-risk low-income patients within Monmouth and Ocean Counties in order to help them reach their health goals. CHWs act as caring neighbors to help patients address the social, medical and community problems that lead to poor health. The goal is to help patients deal with the `real life' issues that keep them from staying healthy, help with the tasks of getting medical care, and work on health goals by doing things like planning medical visits and healthy meals.
Responsibilities:
A day in the life of a Community Health Worker at Hackensack Meridian Health includes:


1. Meet patients in the primary care clinic or hospital and conduct needs assessment, including assisting patients to set health goals.
2. Provide patients with emotional, informational, and instructional support.
3. Make weekly follow-up calls and home visits to patients.
4. Motivating patients to meet their health goals.
5. Provide resources to help patients with social issues like homelessness, substance abuse, and food inequity.
6. Assist patients with organizing their records, making follow-up appointments, and filling their prescriptions.
7. Connecting patients with resources to help complete applications for benefits like insurance and food stamps.
8. Educating patients and partners on PrEP.
9. Finding patients lost to care.
10. Work with other CHW¿s and staff to create a directory of community resources.
11. Document each patient encounter in detail.
12. Prepare reports and documents as needed or requested.
13. Attend regular team meetings.
14. Attend paid training programs.
15. Other duties and/or projects as assigned.
16. Adheres to HMH Organizational competencies and standards of behavior.

Qualifications:
Education, Knowledge, Skills and Abilities Required:

1. High School Equivalent degree.
2. Proficient in Google and computer data entry.
3. Excellent oral and written communication skills.
4. Ability to carry out written communication skills.
5. Ability to exercise judgment in the application of professional services.
6. Comfortable with home visits and outreach.
7. Comfortable with computer data entry.


Education, Knowledge, Skills and Abilities Preferred:

1. Long-term resident of Monmouth or Ocean County with a vast knowledge of social service resources available for these communities.
2. Prior experience as an outreach worker is a plus.
3. Prior experience working with HIV patients is a plus.
