This position is based at UNC Health Care at WakeBrook. WakeBrook (WB) is a behavioral health facility located in Raleigh, NC that offers a continuum of services for people with mental health and/or substance abuse disorders. WakeBrook serves individuals regardless of ability to pay for services. UNC Health Care is committed to establishing WakeBrook as a center whereby novel primary and behavioral health interventions can be tested and then disseminated to other counties throughout the state. WakeBrook currently consists of 2 buildings that were built in 2011 that house 4 separate and distinct services, including: Crisis and Assessment clinic (CAS), Facility Based Crisis service (FBC), Alcohol and Drug Detoxification Unit (ADU), and the Inpatient Unit (IPU).
WakeBrook also has a primary care clinic whose goal is to provide primary care services to people with serious mental illness (SMI) who have or are at high risk for co-morbid medical conditions AND who do not already have a regular medical provider in the community. The WakeBrook primary care clinic is uniquely situated to function as a medical home and integrate medical and behavioral health care services for patients with SMI.

The FIT Program Community Health Worker will provide outreach, counseling, navigational, and case management services to patients recently released from prison into Wake County with a diagnosis of a serious mental illness (for example, schizophrenia, schizoaffective disorder, or bipolar disorder) in addition to chronic medical conditions and/or substance use disorders who are referred by prison health providers prior to prison release. Patients will receive psychiatric and primary care services at WakeBrook.

Minimum Education and Experience Requirements
Demonstrated possession of the competencies necessary to perform the work.
Required Qualifications, Competencies, and Experience
- Willingness to work in various environments, including prison/jail settings, shelters, and home visits.
- Knowledge of information, education, intervention and/or referral services to various populations and/or community health worker certificate/CASAC.
- Comfortable working with a variety of populations, including an array of formerly incarcerated people including persons charged with sex offense.
- Manual dexterity to operate office equipment, including keyboard, copier, telephone.
- Must be able to view and work on a computer for a long period of time, ability to use Microsoft Office and internet.
- Must be able to work in stressful environment.
- Ability to work flexible job hours.
- Valid NC driver’s license and access to reliable transportation.
Preferred Qualifications, Competencies, and Experience
- Experience working with health care systems, mental health and substance use treatment programs, community-based organizations.
- Experience within the past 5 years working as part of a multi-disciplinary team, collaboratively practicing with therapists, psychiatrists, PCPs, health workers, health educators, and/or other health professionals.
- Experience with client assessment, treatment planning, conducting interviews/investigations, advising/counseling clients, and participating in health promotion and health education activities.
- Excellent organization, follow through, and ability to juggle multiple priorities in fast-paced environment with multiple collaborators.
- Excellent verbal and written communication skills.
- Bilingual English/Spanish.
- Experience living or working in Wake County, NC.
- Personal history of incarceration or previous experience interacting closely with individuals who have been incarcerated.
