Community Health Worker

Sheltering Arms Institute is seeking a Community Health Worker to join the team in a grant funded position with benefits for a 12 month appointment term. The Community Health Worker provides valuable services to the population served by coordinating continuum of care services including health, social services, and community and fitness opportunities across the recovery journey. The CHW serves as a liaison/intermediary between health/social services and the community to facilitate access to services and improve the quality-of-service delivery.

Qualifications and Skills

• Knowledge of the community served including people with neurological conditions
• Basic computer skills
• Basic knowledge of medical terminology or health literacy
• Ability to establish and maintain a positive working relationship with care team and other organizations, good communication skills, and ability and willingness to provide emotional support and encouragement to patients.
• Strong communication and customer service experience

Education/Certifications

• Hight school diploma or equivalent
• BSW, BS, CTRS, Health or Human Service Degree preferred
• CHW Certification preferred
