STATUS OF POSITION: Full Time

Position Summary

Community Health Workers (CHW) are frontline public health workers who have a close understanding of the community we serve. CHW will be responsible for fostering trusting relationships between health/social services and the community, thereby serving as intermediaries to patients and their families, helping to navigate and access community services, other resources, and adopt healthy behaviors.

CHW will employ an integrated approach to care management and community and will build individual and community self-health management competencies by growing individual/community health knowledgebase and promoting self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.

Requirements/Qualifications

  • COVID-19 Vaccine required.
  • High School Diploma or its equivalent.
  • Successful completion of a Community Health Worker formal training program institution is preferred.
  • Knowledge of some medical terminology preferred.
  • Basic computer skills.
  • The Community Health Worker will have demonstrated experience in the field. Possess strong interpersonal communication skills; be patient, outgoing, and sensitive.
  • Has working knowledge and understanding of Community Health Net Strategic Plan and Erie County needs assessment.
  • Have cultural awareness and competency related to race, sex, gender, language, and religion.
  • Experience working with adults who have HIV, a mental illness, substance abuse, a criminal record and/or health concerns preferred.
  • De-escalation and crisis management skills.
  • Harm reduction philosophy.
  • Demonstrated critical thinking, problem-solving, effective communication, and time management skills.
  • Demonstrated ability to work effectively in an interdisciplinary team format.
  • Excellent analytical, customer service skills to facilitate care plans and the resolution of case management problems.
  • Ability and willingness to provide emotional support, encouragement, and motivation to patients.
  • Develop and draft goals to address barriers to healthcare.
  • Work with patients to reach goals identified.
  • Identify environments potentially requiring notification to authorities (domestic or child abuse).
  • Reliable transportation.
  • Valid PA driver’s license.
