SUMMARY Under the direct supervision of the Health Marketing & Outreach Manager, the Community Health Promoter/ED Liaison conducts community outreach that results in new patients at Aunt Martha’s health centers. The position provides in-person assistance to help patients navigate health services and apply for medical benefits. This position requires independence and flexibility.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Outreach
- Execute marketing and outreach activities that result in new health center patients.
- Assist in the planning, marketing and support of special events.
- Cultivate relationships with community organizations (i.e. schools, churches, human service providers, community medical providers, etc.) that result in new patient referrals
- Develop relationships with hospital ED staff to offer discharged patients with follow-up appointments and schedule them accordingly.
- Ensure participation in appropriate marketing and outreach activities.
- Represent Aunt Martha’s at health fairs.
- Make presentations to community organizations (i.e. schools, churches, food pantries, human service providers, community medical providers, etc.).
- Educate prospective patients about services provided and model of care.
- Facilitate access to health services by scheduling patient appointments in electronic health record.
- Advocate for individuals and community health needs.
- Maintain daily/weekly/monthly reports on all outreach activities.
Enrollment
- Identify and screen all uninsured patients who may be eligible to apply for medical benefits.
- Determine eligibility and facilitate enrollment in affordable health insurance coverage through Medicaid or Health Insurance Marketplace.
- Process on-line/paper applications for eligible patients for available benefit programs. Programs may include but are not limited to Medicaid, Illinois Health Insurance Exchange, All Kids, FamilyCare, Moms and Babies and CountyCare.
- Promptly process/submit/scan/mail completed applications for processing.
- Follow-up on all submitted applications to confirm acceptance or denial. Research denied applications and follow-up with patient, resubmit applications when applicable.
- Assist patients with understanding Medicaid communications, including steps in selecting a Managed Care Organization (MCO) or Primary Care Physician (PCP).
- Assist patients with Medicaid Redetermination and Marketplace Open Enrollment activities.
- Assist network with verification of insurance eligibility/benefits.
- Maintain accurate daily/weekly/monthly reports on all applications processed.
General
- Must be willing to travel and to work some evenings and weekends.
- Maintain patient confidentiality at all times per the policies and procedures.
- Maintain ethical behavior at all time per the policies and procedures.
- Complete all Core Curriculum training(s) required for this position.
- Perform all other duties as assigned.

QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
EDUCATION and/or EXPERIENCE
High school diploma or general education diploma (GED) required. Two years of relevant sales or marketing experience preferred.
LANGUAGE SKILLS
Strong written, verbal and visual communication skills. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of clients, stakeholders, or employees of organization.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
PERFORMANCE IMPROVEMENT
Ability to understand and demonstrate Aunt Martha's 4-step Performance Improvement process, utilizing the methodology Plan, Do, Study, Act (PDSA).
JOB KNOWLEDGE
Ability to use technical skills and apply knowledge necessary for this position.

Ability to demonstrate accuracy and thoroughness while performing the duties of this position.
