Under the direction of the Healthy Living Director, the Community Health Data Specialist position will be responsible for all aspects of Diabetes Prevention program functions including intake calls, setup of classes requiring communication between centers and coaches, class supplies etc. The Community Health Data Specialist will also be responsible for all data entry for all of the following programs: Diabetes Prevention Program, LIVESTRONG, Enhance Fitness, Blood Pressure Self-Monitoring, Pedaling for Parkinson’s, Y Weight/Gift, the Veterans Program and any others as necessary. The data specialist will pull reports as needed for stakeholders, including monthly DPP BayCare reports. The data specialist will track participant progress and report to physicians on individual participant progress via standardized letters created by HLD. The Data Specialist will gain certification in all health programs as trainings are available in order to coach/teach/facilitate classes as needed.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Member Services and Engagement

  • Call all BayCare referrals for DPP, Enhance Fitness and any other added programs within 48 hours of receiving referral.
  • Enroll participants in the DPP program following through until ending program.
  • Handle all program registration and transactions for all health programs.
  • Follow up on surveys and testimonials from participants in all programs.
  • Attend health fairs and community events to represent the community health programs; as needed.

Operations

  • Complete registration for DPP in YMCA membership system and in DPP data systems.
  • Create program rosters for all programs and track completion as well as membership conversion.
  • Input and track data for ALL programs (Use of ODES, RedCap, Solera, WellD, Active and any others).
  • Assist centers with graduation supplies such as t-shirts, gift bags, certificates which are housed at the Association office.
  • Create and submit program information forms to the YES Center for all upcoming health programs
  • Create reports as necessary for director and stakeholders.
  • Monitor email, properly correspond and follow up with all parties necessary.
  • Attend all sessions for DPP and any other sessions to complete collection of paperwork
  • Follow association dress code
  • Follow assigned work schedule, reports to work promptly, makes arrangements for coverage during absences, and keeps supervisor appropriately informed in a timely manner.

Safety and Risk Management

  • Ensure adherence to all YMCA policies and procedures regarding the safety, health and welfare of members, program participants, guests and staff.
  • Maintain HIPAA compliance in all functions
  • Aid in monitoring coach compliance of HIPAA and other risks

Education/ Experience Required:

  • High School Diploma, or GED required. Associate degree preferred.
  • One to two years of experience administrative setting.
  • Must have a minimum of two years of data entry experience.
  • Deadline driven, very organized, attentive to detail, with excellent communication skills (written).
  • Bilingual (Spanish) preferred.
  • Demonstrated ability to multi-task and adapt to changing contexts and priorities
  • Ability to work independently and with minimal supervision.
  • Ability to multitask and prioritize work to meet daily deadlines.
  • Must have high level of confidentiality to handle sensitive and confidential situations and documentation.
  • Excellent interpersonal skills; ability to relate to individuals at all levels.
  • Excellent customer service skills.
  • Proficiency in Microsoft Office (specifically Excel and google spreadsheets)
  • Must be extremely organized, detailed with excellent follow through.
  • Excellent analytical skills.
  • Strong commitment to service.

Certifications/Trainings Required:

  • Must obtain within 30 days of employment and then maintain current certifications in CPR/AED and First Aid.
  • Maintain other required certifications as stated in the training matrix.
